To ensure a safe, efficient, and compliant training experience, all students are required to review and complete the documents below prior to attending class.

What You Need to Complete

Before your scheduled class, please review and complete the following:

  • Liability Waiver & Release Form

  • Training Agreement / Student Contract

  • Maryland State Police Required Forms

  • Qualification Sheet (if applicable)

Failure to complete required documentation may result in delays or inability to participate in your scheduled training.

Additional Requirements

In addition to completing your forms, please ensure you bring:

  • A valid government-issued photo ID or passport

  • Fingerprinting completed prior to class

  • Any required documentation related to your permit or application

  • Students must meet all legal eligibility requirements under Maryland and federal law.

  • Incomplete documentation may result in rescheduling at the discretion of the instructor.
